HOW it works

Cooperative Functional Medicine Institute's expansive team of qualified caregivers, including Health Coaches, Nurse Practitioners, and Physicians, will be a part of your treatment model from the beginning to the end of your health journey until you graduate from your illness/meet your goal. We will be taking your personal healthcare data and will have a deep dive into your activities of daily living (ADL), which will be analyzed by our team of clinicians. ADLs involve activities of work, leisure, sleep, etc.
